Robotgeassisteerde PSMA-radiogeleide chirurgie bij recidiverend prostaatkanker met de DROP-IN-gammaprobe

Samenvatting In dit prospectieve, in vivo haalbaarheidsonderzoek met 20 patiënten recidiverend prostaatkanker is onderzocht of een geminiaturiseerde DROP-IN-gammaprobe mogelijkheden biedt voor robotgeassisteerde op PSMA gebaseerde radiogeleide chirurgie (RGC) (NCT03857113). Patiënten ≤ 3 prostaatkankerrecidieven het kleine bekken (lokaal lymfeklier) PET-CT-scan na primaire behandeling werden geïncludeerd. Na intraveneuze toediening van 99m Technetium-gelabeld PSMA-ligand ( Tc-PSMA-I&S) vond de PSMA-RGC plaats. Primair werd haalbaarheid onderzocht. Met behulp DROP-IN-probe konden 19 21 (90%) preoperatief geïdentificeerde laesies gereseceerd worden. Op laesieniveau bedroeg ex-vivosensitiviteit 86% en specificiteit 100%. Bij één patiënt trad Clavien-Dindo-graad V‑complicatie op. Wij concluderen dat mogelijk maakt. deze procedure intraoperatieve detectie resectie zowel nodale als lokale mogelijk.
